It doesn’t really fall off my shoulders but when it moves a bit, I just adjust it back. It’s also a habit of mine to pull it down, especially the knot where both ends are tied

What I would do to have smaller shoulders! Lol
What you can probably do is to make your knots when you tie both ends together closer to the center of the scarf. That way, it’ll be more snug. Plus, you’ll have a bigger knot that can add flare!
