Yes, they have a form you fill out and I had to attach my order confirmation (it was a Coach order).
Keep on eye on E-bates. Several times I've had to email them and tell them to give up the discount period. I never got a penny.
The last email I got when it happened was really rude too. They said if they don't get paid they aren't paying me. Um, not my problem. If you have an issue with getting paid by the stores, deal with them. I played by the rules, pony it up and then go see Judge Judy for the other side.
