Ladies in the facebook facial fat graft group have shared with me how German, UK and US surgeons are having bad results with facial fat grafts and now some don't even recommend it anymore.

I put together a collage of Dr Hyo Seok Seo's facial fat graft. I had it done twice with him in 2018 and 2020. I didn't need the top up in 2020 but wanted the round youthful forehead which he gave me back in 2020 so I had the top up and I now still have a rounded youthful forehead in 2024.

I know what you are thinking...I looked better with black hair! lol. I have been debating this but can't recall the box colour dye I used back in 2018!

Hi MissOrange, I have been reading through your plastic surgery journey and you seem very knowledgeable. I am 54 would like my youthful appearance again. I have an consult and surgery appointment in April with Dr. Seo at Machimpyo to have DES, sub brow lift, face fat grafting, and thread lift. What advice can you give regarding aftercare for these procedures? Do you recommend that I take Prednisone for 1 week? Did Dr. Seo prescribe Prednisone when you had your procedures? I am currently on a keto diet (high fat, high protein, low carb), so do you recommend that I stay on it even after surgery? Thank you for all your helpful information!

Hi @t2weati, no Dr Seo does not prescribe prednisone. With fat graft and thread lift you will have double the amount of facial swelling and the pain will be immense. Bear in mind Korea only gives you tylenol for pain so you may need to bring some stronger painkillers but are limited by the laws on what drugs you are allowed to bring into the country. I would suggest you find a facelift surgeon instead of get temporary threads. Dr Seo is a specialist at DES and facial fat grafting but he does not do smas facelifts.

Hi @MichiD, never judge a result at 10 days. Laxity occurs at 4-6 months.

@Tremere, Dr Dongman Park at the Bio clinic does extended deep plane FL for $15,000-$20,000 and has outstanding b&a results on Instagram. If there were no covid quarantine for arrivals to Korea, I would say fly to Korea. There is also Dr Lee at ASPS in Korea renowned for facelifts and a real Asian American lady posted her journey with Dr Lee on this forum.

If you cannot wait for Korea to open to foreigners without mandatory quarantine, then best to explore surgeons local to you. I have not found a patient of Dr Karri’s to chat with and in my opinion his photos are not as good as Dr Park or Dr Lee. But if cost is an issue, then he is the cheapest for deep plane at £7500.

Dr Michele Pascali in Rome has good Instagram results for deep plane facelift for around €15,000.
Miss Orange, have you heard of this surgeon in Turkey? He performs vertical facelifts

Today’s post is on SVF or stromal vascular fraction injections (used in microneedling) in lieu of proper facial fat transfer (which involves harvesting autologous fat, using prp fat and injecting in fine layers which is more time consuming and requires more expertise). You got it, I do NOT recommend SVF which is an artificial product or laboratory manufactured product. As this is made up of fat cells from a different donor, it is not autologous so more likely not to take.

Please do not go through life saying I will trust my surgeon 100% and get whatever he suggests. At the end of the day it is your body and you have to live with the consequences of a silicone implant or SVF microneedling. Don’t cut corners. Find a good experienced facial fat graft surgeon to build up your weak chin or to give you baby skin with autologous prp fat graft.

Hi Miss Orange. What do you think of this method of bulbous nose reduction? Do you think it’s effective for people who don’t want a big change?

I have found more information on SVF. It is something pushed by Dr Comert in Turkey. I have taken Comert off my list of recommendations as 1. he does not prescribe postop steroids, 2. he pushes svf comprised of donor fat cells from obese people, diabetics or normal people sold in a vial with other components 3. he has no 1Y+ facelift updates on his IG and 4. he just told a British lady that her eyes would be swollen shut for 2 days (she is getting lower bleph, possible fat transposition, transtemporal midface lift and DP FL/NL). He may be cheap but I have been following a British lady who had her surgery with him yesterday at 1 pm Turkey time over 27 hours ago and she has made no contact with her friends or any updates to her IG detailing her journey. I just hope she is okay.C76266F5-7588-45BC-B2F0-374D5A0E39B9.png

So in Korea, many places can do SVF by your own fat cells, so it's analogous, and wouldn't have the issues you mentioned of donor fat SVF. I wouldn't want donor fat derivatives either. But your own fat SVFs, maybe...

A tpfer requested a video from me with no makeup and showing the scars at 2 years so I complied. I then thought why not make a public video as I keep banging on about judging a result from a patient video at 1Y+.

Not sure how long I will keep my IG account public. I keep changing the settings, paranoid there are bad people out there looking at my family! lol.

Hi @MissOrange - can you explain the "pinch test" that you did in your Instagram video? Is it a way to tell whether or not a facelift was effective? And would one pinch the skin outwards (towards the ears) rather than inwards?

Hi @lisa.t, I use it to gauge whether one needs a facelift by pinching the skin to the side or downwards to see if there is at least an inch of loose skin. Nick says there needs to be at least 2.5 cms of loose skin on each side for him to do a facelift. And the other reason is to do it afterwards to see how tight the facelift is. If one cannot pinch any loose skin then the facelift is a success.

I have finally received news via a third party on my friend's condition in Turkey. She was taken to theatre for her revision DP face and neck lift with Dr Mehmet Comert at 1 pm on Tuesday. She did not return to her hospital room until 4 am Wednesday! She was panicky, swollen and sore. Her eyes are swollen shut and she was unable to call her mother who had been worried all this time along with all her friends. This suggests Dr Comert does not give intraop dexamethasone to reduce swelling. I am now crossing off Turkey. You may save money but swelling may equate to future sagging and it is a long way to go if something went wrong.

My facelift surgeon took 2 hours for my first facelift and 90 minutes for my retightening 2nd facelift. To be under anytime from 1 pm to 4 am for over 12 hours may put one at risk of a dvt or pulmonary embolus. Another reason not to get deep plane face and neck lift as your first facelift as revision DP is super difficult trying to differentiate white scar tissue from white branches of the facial nerve.
