I did (finally) get a QB offer recently at Madison, but for context, it was my first of the year, I made my list back in February and I've been a pretty consistent shopper throughout the year. I know people will ask - I haven't done the tally, but I would guess my pre-spend is ~2-3x NOT counting a somewhat sizeable (>5k) non-QB bag purchase. My impression is my pre-send had a bit less to do with it and what helped is that I'm a repeat consistent customer that will continue to shop with my SA for years to come. I also have a pretty picky list (I'm basically a K28 classic colors girl), so not the most flexible in what I'd accept and I think my SA knows that

Is Kelly Elan considered a QB in NYC stores? My parents have just moved here and I’m London-based but visit often, but would prefer to stick to my London store re:spend.

Basically 100%. Unfortunately.

ETA it’s not a quota bag but that doesn’t mean it is easy to get nor freely offered. It will require an ongoing spending history and relationship with an SA.

The Kelly elan is not a QB in NYC but agree with @acrowcounted that it would be highly unlikely that you would be offered this bag as a walk in. This would definitely require an SA relationship and some level of purchase history. (I have no idea how much. I got mine from my SA in between QBs.)
It also seems like the Elan is getting more popular now so probably harder to get than when it first came out.
