BASIC PICTURES REQUIRED (but more may be requested): If necessary pictures are in the listing, it's not necessary to upload and duplicate them.

• Front of item

• Full clear and legible creed text and serial number
• Made in tags (when available)
• Measurements
• For bags with turnlock closures, show pictures of back of female side of turnlock
• For bags with magnetic snaps, show pictures of the male part of the snap so that the numbers and letters on it can be read

Saw NWT one on ebay with FS3406 as the style...correct? Any idea of age?

It's authentic and you're correct in that it appears to be missing the checkbook insert.

I'm guessing it's from about 2004.

Hi, Can you tell me if this is authentic? Thank you.

It seems to be ok. But since there's no serial or style number - which is NOT a problem since it's a smaller bag and considered an accessory - it doesn't have to have either one - it's hard to know what style it is since Coach made quite a few different versions of this design and fabric.
