Hi MissOrange, thanks for this thread. It’s by far the most informative one on face lifting which I can find online. i did SMAS lifting a week ago at ASPS Seoul (I also enquired with V.lift but bet on ASPS ultimately) and just want to contribute my experience here. Unfortunately I still swell quite badly despite taking prednisolone which I sourced from my GP for eczema. I started off with 10mg pre ops and increase to 20mg per day from D1 to D4. Not to mention I bruised badly too. Dr Lee Min Koo, the chief surgeon assured me that the surgery went well though so i shall wait and see in 6m time! I have alot of bad experiences with Seoul PS clinics despite its world class reputation for PS (Banobagi is the worst in my experience) since starting my PS journey since 2017 and I am hoping for the best this time that no revision is needed.

Another thing to add on, when I consulted online with V.lift regarding swelling concern, their reply was as below

“The steroid we mix is dexamethasone, this is the steroid mostly used in this generation now :smile: Nevertheless, the important thing is there is a mixture of steroid in the de-swelling injection which will definitely help with swelling!”.

I didn’t go with V.lift in the end so not sure how true is their words but ASPS for sure doesn’t give steroid injection.

Today's post is on the 'fallen' eyebrows and why not to get a brow lift.

Dr Timothy Miller, double board certified facial plastic surgeon of Refreshed clinic in Florida, describes it best when he says a brow lift will only make the eyes look more hollow as it does not address the loss of facial volume (fat, bone, collagen) as we age. He quite rightly advocates lid fat transfer and upper bleph instead of a brow lift.

Here is 59 yo Brooke who desperately needs full face fat graft to replenish lost volume.

I would take it one step further and say instead of a forehead lift which when it sags creates a shar pei dog forehead with multiple forehead wrinkles and folds, one should instead get forehead fat graft or full face fat graft in Korea to replace lost volume like 57 yo moi and 42 yo Han Ye Seul have had.

If you need botox for forehead wrinkles it suggests you are already losing facial volume and eventually may need fat graft to replace lost bone, fat and collagen volume. I have never needed botox for my forehead since getting fat graft 8 years ago.

I have noticed my muse for anti aging is Han Ye Seul as she has the closest face shape to mine. Bearing in mind our 15 year age gap, I am nowhere near as good looking as she but it helps to see what I need next and this collage shows how aging my neck now looks.

HYS's face is a lot smaller than mine as she has had zygoma reduction and V line surgery with chin elongated with bone, procedures I cannot have as my face would reject titanium plates and screws. Riding the Seoul subway earlier this year I noticed more ladies with smaller v line faces. It is definitely noticeable as it makes the face small for the body yet the small face is very aesthetically appealing. I tried not to stare but was amazed at how small bone contouring can make a face.

Should ones bodyweight be a factor when determining the outcome? Would leaner individuals have a better / sustainable outcome than someone who is carrying more bodyfat (esp round the face)?
Hi @zet88, absolutely weight plays a huge factor especially in the face. Fat weighs down the face so facelifts are less likely to be successful in the longrun as the natural fat in the cheeks will create nasolabial folds and the weight of fat may drag down the face and create more neck folds as time goes by. The best outcomes are in lean individuals as you surmised, as there is limited weighty fat. I would strongly advise ladies to be at their ideal weight before paying £20,000+ for a facelift as when one loses weight, the face may sag also.

@MissOrange, I hear you about not doing a browlift and doing fat transfer to the eyelids when you have hollowness on your lids.

However, there are people such as myself who do not have hollow eyelids but instead I have excess skin and fat on my lids, which make the lids bulge out instead of being sunken. I think in this case having a fat transfer to the lids would exacerbate the problem, no?

So looking at my eyes (pls excuse the lash extensions - I hope you can see the issues and ignore the lashes):

1) I do indeed need an upper bleph.

2) The distance between my brows and my eyes is already quite short. After the upper bleph, the brows may be pulled down further, thus making this distance even shorter.

3) Due to ageing and a stupid threadlift with no steroids post-op, I have excess loose skin on the outer corners (all the way from the end of my brows down to my outer eye corners) - pls refer to the second pic where I've circled it in red. This is making the skin on my outer eye corners even more excessive, and also at the same time making my lateral/outer brows even more droopy. As such, I do think that I would benefit from a temporal or pretrichial browlift (ie. not a direct browlift) to lift & cut off this excess skin and lift up my outer eye corners. I note also that my brows used to be more arched upwards.

4) A fat transfer to the temples and perhaps under the eyes too would definitely help reverse some of the ageing.

5) I will also ask about fat transfer to the rest of my face (granted you can't see my whole face here), like you've done. Agreed that full face fat transfer - when done well - really helps the face to look naturally younger (ie. not a chipmunk face as fillers can do).

What would be your thoughts and advice please?

I will be updating my IG with daily throwback videos of my £3495 2022 facelift to show the public the importance of 1 week of postop steroids to stop swelling and how you must not jump into an invasive deep plane first.

I have seen too many swollen alien faces in ladies who did not take postop steroids end up months later as sagging faces with prominent nasolabial folds.

Then the surgeon may gaslight you and say deep plane facelifts do not remove nasolabial folds. How can they when deep plane severs all your facial retaining ligaments, the scaffold that holds up your face! Don't be fooled by swelling holding up your face in the short term. Then they may suggest a mid facelift. Then you pay more, swell, deswell and sag. Then you run out of money and live with the sag and nasolabial folds or resort back to fillers.

Stop lining the pockets of male surgeons with gold. Get a smas plication or cheap £5k macs lift with a week of postop steroids. My IG throwback videos are to show you that you can get a great facelift for cheap in the uk but only with postop steroids.

Later this year I will be posting videos as the youngest looking 58 yo grandmother!

Hi dannythedragon, here are the photos - before, D2 with swelling at its peak, D7 (i have really bad bruises and my incision line start from top of ear all the way to neck) and D8. I will be taking down my photos after some time. I was hoping the incision would start from around temple area so perhaps it would kinda "elongate" the length of my eyes but the doctor refuse to do it on me.
