Hi @beepboop4, after the face deswells, the grafted fat in the cheeks may drop to the lower face in 4-6 months or the fat reabsorbs leaving the face sagging. It is harder to maintain the fat in the upper cheeks as fat is heavy. Think of breast fat transfer and similarly it is hard to keep the fat graft in the upper pole as it drops due to the weight of fat. I recommend facial fat graft in the forehead, temples and upper eyelids but when it comes to the cheeks and lower face it is very unpredictable. I have had to have numerous repeat fat grafts to my cheeks and lower face as they reabsorbed unevenly, my lower face became sunken and my face sagged. Eventually one retains enough in the face and a lower facelift lifts up the fat graft and repositions in the cheeks better. In other words, once you have fat graft to the cheeks or lower face it is hard work to then maintain or try to achieve a lifted look as fat is heavy and if one gains weight, the fat hypertrophies and drags the skin down and causes nasolabial folds to appear. The lower face loses fat graft faster as we move our mouth when we eat and talk so a full face fat graft may end up very uneven and need multiple attempts to fix it. Facial fat graft in the cheeks and below takes a lot of persistence and repeated grafts to achieve longevity. I’ve actually got too much fat in my face in my honest opinion. But with aging, the facial fat will slowly go down so hoping I can have a slim face in my 60s. I always wonder what I would have looked like had I not had fat graft in my lower 2/3rds of my face and just stuck to having it in my upper third.
Dear MissOrange love your thread and I’ve been following your thread for years. If it takes repeated grafts I’m wondering why not go for filler like Ellanse? Are the results of facial fat transfer vs filler superior in terms of helping one to look more youthful?

Hi @Minda, thank you for following my thread for years! It is good to get feedback. Fillers are not permanent. I had dermal fillers in my 40s and had to have them every year in my nasolabial folds. Also one cannot inject fillers all over the face as the body may react to the overwhelming amount of foreign substance and after menopause one needs to replace a lot of facial fat loss. Fat grafting eventually becomes permanent with repeated grafts and I think the fat grafting is one of the main reasons my skin texture is baby soft, an added bonus.

I love how my Korean surgeon Dr Hyoseok Seo calls it Natural Baby Face Surgery. He truly is the best in the world at facial rejuvenation with fat grafting. He filled my forehead, temples, eye hollows, cheeks, smile lines, and chin with top ups until it was permanent and of course did my upper bleph too. Amused he calls the upper lid hollow the eye hole. My youngest sis now says she wants fat grafting to her upper lids like I had as my eyes look good. Not sure how to book him without MVP manager Ellen as he runs his own clinic now. I could be a model for his clinic! lol. Actually Ellen the manager of MVP used to tell me Dr Seo owed so much to me as many patients came to Dr Seo at MVP because of me. I was just happy to have a Korean meal with Ellen and the staff and other patients in years gone by.

Yesterday my 58 yo American high school classmate asked for my secret to not ageing a day. Thank you Dr Seo! He rebuilt my sunken menoface. Maybe I should lead a tour group straight to Dr Seo’s clinic! lol. Then a tour bus full of younger faces would leave his clinic and hotel. I must say the eyelid surgery with lid fat grafting gives immediate results and knocks off 30 years in one afternoon! The full face fat graft needs weeks to recover from the swelling and bruising.

Here is a 4 year update on my arm lipo for €1250 at age 53 by Dr Bert Oelbrandt in Belgium. Huge difference between the before pic with batwings and the 4 years after last month at age 57 in the same bikini. Thank goodness lipo was enough as an upper arm lift scar is really long! Always go for lipo first.

Hi @naimal, use of drains depends on the surgeon, the amount of bleeding and extent of resection. Most major operations use drains.

Do you think chin fat graft is the way to go to lengthen the chin or the fillers? I will be getting my facelift next Wednesday along with fat graft on my temples, and deciding if I should also get it on my chin. You are my yellow book of PS!!! Hi @sunny23, good luck with your facelift and fat graft to your temples. Who is doing your fat graft? Dr Seo put fat graft in my chin as part of a full face fat graft to create a diamond shaped facial fat graft pattern. I would say fat is more permanent than fillers.

I'm getting my facelift by Dr. Shim at Vlif, so I'm going to ask them to do it at the same time. Would you recommend doing it on my chin to lengthen as well? Can I DM you?

Hi @sunny23, have you followed my rule of finding 3 patients who have had a facelift with the surgeon and checked on their results after a year to see if the facelift held or dropped? It is a lot of money to base your decision on early results at 2-3 months when the soft tissues have residual swelling that may hold the face up. I see from realself he only has one review and her postop photo shows +++ swelling which has me concerned that this may cause the skin to stretch and drop after 6 months and may indicate she did not get steroids either intraop or postop. Her after photo is only at 3 months and quite frankly I can’t tell which is her before and after photo. Personally I now would never have a facelift without a week of postop steroids. Too often ladies are lured by early results which are always good and forget to find results after a year to see if it holds. Also what is his revision policy? Are you prepared to fly back to Korea if it drops after 6 months? Does the price include one free revision within a year?

It has been 7 years since I had revision double eyelid surgery (for my triple folds/wrinkles and eyelid hollows) with ptosis correction and eyelid fat graft with Dr Seo and amazingly the eyelid crease has held! I spent £2-3k every 2 years in the UK with Dr Stanek and Dr Viel for upper blephs as the crease kept dropping! Now I know that they may have only performed a skin only excision bleph and not addressed the muscle. I am so pleased I chose Dr Seo to rejuvenate my aging eyes. Back then the won was weak and the pound strong so I paid £1668 which is extremely good value for young, beautiful eyes that have lasted 7+ years! I like how Dr Seo now has a page on his website dedicated to triple fold and explaining why this happens. He truly is a master of eyes. I did not know he performs over 1000 eyelid surgeries and revisions per year! Don’t go to one plastic surgeon to do everything. This was my mistake as I went to Dr Stanek for everything in the past rather than choose a different surgeon for each procedure. Dr Stanek’s talent was actually for rhinoplasty. Each surgeon has remarkable expertise and talent in one particular operation. Seeing is believing. You won’t get a 7 year eyelid postop follow up anywhere as the lid creases drop with other surgeons! I am so glad I chose a surgeon who performs 1000 eyelid surgeries a year!

My facelift surgeon Mr Nick Rhodes has just uploaded on IG a 1 year postop of a 70 year old lady he did upper bleph and a full facelift and necklift! Wow! His prices are £1995 for upper bleph and £10495 for a full face and neck lift. I love how he is very transparent with pricing and updates it on his clinic website. I hope he is still operating when I turn 70! But as he is my age he may be retired by then. Sigh.

In Korea, the pricing seems so random and a lot more for foreigners and there is no guarantee they will revise for free up to a year later. Nick Rhodes offers free retightening up to a year post his facelifts if needed.

When I had surgery at VLIF I was told the post op IV “deswelling injections” had steroids as part of the mix. Definitely seemed effective for me.

I also practiced anti swelling strategies that definitely helped (ice on and off for first week / 10 days - not just first couple of days, wear compression as recommended with frequent breaks over first couple of weeks, low salt / sodium diet, gentle walking daily, sleep upright, gentle lymphatic drainage massage to clear lymph buildup in tissues, etc.
