That is her. She has always had those low ears. She has had a lot of plastic surgery on her face, and I suspect she has had some recently. As for the weight loss....sorry to be cynical, but it's drugs. She's mentioned before that the only time she is ever skinny is when she is addicted.
She definitely had a recent nose job. I still can't tell if this is her or not, though, even comparing with recent pics. Those sunglasses really hide this woman's face! There are a bunch more pics from this shoot at www.hollywoodrag.com
she's had some pretty extensive cosmetic surgery (I read that in a magazine a while back). Her facial features are not the same as we are used to..

Her nose and lips are smaller. That's unusual in Hollywood where they get smaller noses and bigger lips. I like her 'back to normal' lips.
Yeah, she's unrecognizable as Courtney in the MSN picture but at least her lips don't look like inner tubes anymore.
When will this woman STOP with the plastic surgery???
As for the thinness, it's gotta be addiction of some kind.
Well, Courtney's "secret" weight-loss technique isn't so secret, but is, unfortunately, dangerously unhealthy. My guess is that she is back on "the Horse." On a side note--eww on the Bruce Willis! If he has been playing tonsil hockey with Courtney, he has definitely lowered his standards since breaking up with Demi.
