My understanding, at least for the scarf shirts, is its full custom. They take your measurements and send it to Paris. Paris then makes a Muslin shirt using your measurements and sends that to your store to fit. Madison’s tailor, with the head of men’s MTM and my SA present, checked the fit on me. We made some minor adjustments and then the muslin shirt was sent back to Paris to be used as the template for the silk shirt. Any shirts you order after that will take less time to make because you bypass the first 2 steps since they already have your measurements and the muslin template in Paris. That’s of course assuming I don’t gain too much weight
My silk shirt fit perfectly and did not need any adjustments. As I posted earlier, the MTM manager wanted to see the fit and had suggested maybe putting darts in the back but once we saw it on we all agreed to leave it as is.
I hope you are able to do it through your store. It was a really interesting experience and I was really happy with how the shirt turned out.
Is M2M like a bag SO where you have to be invited? Or can I just let my SA know that I'm interested in doing this?
Fortunately, it’s not at all like a bag SO. Just let your SA know. The main problem will probably be if you can get the 5 scarves necessary to make the shirt. They won’t do a shirt in any scarf that Hermes ever made. You have to pick from the scarves in the look book.

All the silk shirts by far look amazing! Can't wait for mines to arrive. I along with my DH attended our stores MTM event last Oct. His full suit arrived before mines. We both did get our muslins rather quick and adjusted in between though.

I would love a silk lined blazer like that! I wonder if they would make a jacket in a more feminine cut.

The MTM manager had mentioned to my brother that they could make jackets with silk scarf lining. I love the pop of color lining in your DH’s jacket. En Liberte is one of my favorite scarves from last season.
I secretly tried on DH's blazer and it's an oversized fit but somewhat works if that the look I want, just need to roll up the sleeves
So the MTM jacket is not fully custom, he got to pick the fabric/buttons but not the style/fit. Our store only participated in the event but doesn't have MTM regularly. Maybe if a store provides this service year round would be able to do so.
