You can see his sunglasses shadow right under the arm. The line running along his jawline looks like a scar, except it doesn’t appear in any other photo so it probably isn’t really there.
I wasted five minutes looking at that photo. Damn, I am not proud of myself.
I think it's a mirror-like reflection from the shiny top edge of the boat rail in front of him, it goes behind his ear, too.

Maybe the nannies bring the kids with them. In that case, I'll try to believe it's the two kids under ten.

They’ve done a good job of keeping the kids out of the spotlight without being weird about it.
It looks like Amal has lost more weight, perhaps a bit too much? Maybe it's just a bad photo.
