Question - How is it that the Duchess of Alba has all those titles? I'm assuming there are a few that are inherited but the rest? Marriage? Given by the King? Bought?

Also, is there such a thing as a highest title? Is Duchess her highest? Is Duchess above rank to a Princess ?
