Order keeps getting cancelled, need to do phone order

Maybe it has something to do with one of your accounts which was deleted? Did you place any succesfull orders after that? Your scenario sounds to me like you've been blocked (not saying this is your fault), because the CA didn't want to do a phone order for you and nobody seems to want to tell you what is going on, sounds fishy! Is it possible to visit a store nearby to ask? Otherwise I hope a SA can help you out!

It could be the double account issue. I wasn't able to order online, but I found an SA from Charlotte, she just put through the order and I got the bank notification and the receipt from the Charlotte LV store!
