When you picked up from the boutique: February, 2010 (three and a half weeks)
Condition of the bag: (General spa or specific damages?): General refurbishment + stripping and rewaxing the edges of handles and top of flap

Slightly OT but the bag does not seem that old and I am surprised it needed rewaxing. Is this a common requirement after only 18 months?
i am also surprised it needed work done after only 18months. i am even more surprised that they charged you! i would think that for the price of the item that the would cover any needed work caused by manufacturing for at least 2 years. which is now long LV covers their bags.
I can't speak to whether it's a common problem or not, but the wear on my birkin's handles was very noticible and my SA said, don't wait too much longer for this. I'm guessing that most, if not all, of the cost was for the refurbishment, but it's hard for me to say because this is my only H bag and I don't know what this normally costs. I was thrilled with the repair as well as the time it took, and didn't mind the cost, just to have it fixed and back!
$125 would be the cost for just the general spa----not the rewaxing, it looks as though they did that at no cost to you.

When you dropped off at the boutique: January 2009 - I asked if I could keep bag with me while handle was being ordered and made.
When you picked up from the boutique: I think they forgot about the handle (And I forgot about the handle since I had the bag) I remembered in September, reminded them and handle was done by November? 2009; they called me to bring bag in and (two weeks to put on new handle 495)
I also had new shoulder tabs (300?) and new arms (495?) (2-3 months) - I think the total cost for this was 800 roughly (I remembered it originally as 795 for just the arms, but that doesn;t strike me as accurate) but I would have to find the receipt to double check.
