Where can I buy a top like this?

I've seen a beautiful top on a lady in this week's Grazia magazine and I would love to be able to buy something like it - has anybody seen one around or knows where I can buy one - preferably either on line or in the UK please.

The points that I really like about it are the neckline, the floaty'ness' and the cuffs!
