My knuckle is relatively large compared to the base of my finger. So if I get the smallest size ring that fits over my knuckle, it is a bit loose once on the base of my finger and has a tendency to spin. Is there anything I can do to fix this?
 
There is something called sizing beads that your jeweler can solder onto the inside of your ring. They are a couple of balls in the metal your setting is so while the ring might be wider to go over the knuckle but it will sit secure at the base of your finger.
 
If you have a gold (white, rose, yellow) you can get what is called a butterfly or horseshoe spring inside, that will move away from your finger so it can go over the knuckle but when it gets past the knuckle will spring back to keep it tight below that.

But it will depend just how much smaller. I personally have sizing beads in my ring which is platinum. If I could have gold near my skin without a nasty rash, Id have a butterfly. But these work fine, and I am probably 1/2-3/4 a size smaller below the knuckle, though I am not smaller when I wake up or when I am hot as heck from the heat or working out.
 
You can buy adjustable one-size-fits-all rings, usually these are more fashion/cocktail rings than fine jewelry but they are still fun. They are open in the back and you push the sides in and out for a custom fit. A bypass style ring may work well for you too.
