I went to the boutique inside Crystals City Center here in Las Vegas. I had them order my size (which was the last sz 5 in the country). They are available in limited quantities in certain boutiques... your SA can find one for you! They run true to size

Oh wow, I wouldn't have thought about that! So glad to know. I have suede loafers from the 90's with small silver horsebits. I think we were able to specify then? Can't recall.
