Most of Hermes is most likely machined, but hand finished (enamel, polishing, engraving)
Cartier is probably also a combination, as would Tiffany's. I don't believe either does anything 100% hand forged besides "important stones", mostly due to quantity of pieces they have to crank out.

thanks.i was hoping designers still would take the time for every detail but i guess not :/
 
They are more concerned with profit margins. The more important the gem being set, the more time and effort (and cost) goes into it. That said, MOST die-struck and cast settings are pretty soundly made and finished. I'd take a die-struck over most hand-wrought rings any day, because I like machined uniformity and strength to hand-made quirk.
