I find this shocking, I guess the old man must have pulled the train guard out of the open door and the following mutual push/pull they ended up in front of the train (the only door open at the time being the one at the very front, behind the locomotif).

A train guard and a passenger were seriously injured in a scuffle at the station of Dinant (Belgium) yesterday.
The signal for the train's departure had already been given when a man in his seventies tried to board the train. Some sort of argument ensued and both men ended up on the rails, only to be hit by the train as it departed.
The wannabe-passenger lost part of his right foot, the train guard lost one leg just above and the other just below the knee. At this point the guard is in a coma and the next two days are crucial.

No mental problems, just angry that the train was leaving and the guard wouldn't let him on anymore.
