It's 100% completely the officials/state's fault. If it wasen't barricaded up according to regulations and laws, it doesen't matter if she was texting or not, as long as she didn't jump into it volentarily. Holes like these present a hazard in an environment frequented by a lots of peolpe, especially in an urban setting. They're lukcy it wasen't an old lady falling into it.
